Requirements

  • Current LPN license in good standing with the state board of nursing
  • Strong medication-administration and documentation skills
  • Natural leadership-able to guide, teach, and uplift nursing assistants
  • Calm under pressure, organized with shifting priorities, and dedicated to resident dignity
  • Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN training program and possess a current, unencumbered, active license to practice as an LPN in our state.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
  • Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must have the ability, with or without reasonable accommodation, to lift, push, pull of move 50 lbs on a regular basis and 100 lbs occasionally.

  • Previous clinical experience in senior living, skilled nursing, or acute care preferred

Responsibilities

  • Administer medications, treatments, and physician-ordered procedures in full compliance with state regulations and Legacy protocols
  • Maintain accurate narcotic counts and resolve any discrepancies immediately with the Nursing Supervisor
  • Complete focused assessments and monitor residents' physical, mental, and emotional status-promptly reporting changes to the Director of Nursing (DON)
  • Chart thoroughly and on time, reflecting care provided, resident response, and alignment with care plans
  • Update and follow physician orders, diet modifications, and therapy recommendations; flag inconsistencies for review
  • Handle administrative tasks such as medical forms, evaluations, and incident reports with precision and confidentiality
  • Direct and mentor nursing assistants, ensuring daily assignments are clear and executed to Legacy's standards
  • Communicate shift priorities, resident updates, and schedule adjustments at change-of-shift huddles
  • Foster a supportive, collaborative environment-the calm center residents and teammates rely on
  • Create an atmosphere of warmth and personal interest; greet residents by name and celebrate their stories
  • Encourage and coordinate resident participation in activities, therapy sessions, and worship services
  • Serve as a trusted liaison to families, answering questions and providing timely updates
  • Monitor supply levels; restock and reorder to keep care seamless and efficient
  • Ensure resident rooms, treatment areas, and common spaces remain clean, safe, and infection-controlled
