About The Position

"Navitas Healthcare, LLC" is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care in a Skilled Nursing Facility (SNF) setting. The LPN will work under the supervision of Registered Nurses (RNs) to support resident care plans, assist with daily clinical operations, and ensure compliance with facility standards and state regulations.

Requirements

  • Completion of an accredited Practical Nursing program (Diploma/Certificate)
  • Successful completion of NCLEX-PN
  • Active LPN License (Required)
  • Basic Life Support (BLS) Certification (Required)
  • COVID-19 Vaccination (Required; medical exemptions only)
  • Influenza Vaccination (Required; exemptions/declinations allowed)
  • 0–2 years of LPN experience preferred
  • Prior experience in Skilled Nursing Facility (SNF) or Long-Term Care strongly preferred
  • Strong clinical nursing and patient care skills in LTC/SNF settings
  • Ability to assist and mentor CNA staff under RN supervision
  • Knowledge of long-term care regulations and compliance standards
  • Strong communication, teamwork, and interpersonal skills
  • Excellent organizational and multitasking abilities
  • Attention to detail in documentation and medication administration
  • Compassionate, resident-focused care approach

Nice To Haves

  • Advanced Cardiovascular Life Support (ACLS) (Preferred)
  • IV Certification (Preferred)

Responsibilities

  • Deliver direct patient care in alignment with individualized resident care plans
  • Assist RNs in implementing and evaluating nursing care
  • Administer medications and treatments within scope of practice and facility policy
  • Monitor and report changes in resident condition to RN or physician
  • Collaborate with interdisciplinary teams including RNs, physicians, and therapists
  • Conduct patient rounds and respond to resident and family concerns
  • Maintain accurate and timely clinical documentation
  • Support quality improvement initiatives and compliance audits
  • Provide guidance and support to CNAs and junior staff under RN supervision
