Requirements

• Active Licensed Practical Nurse (LPN) license in the state of [Insert State].

Responsibilities

• Collaborate with physicians, RNs, and caregivers to ensure continuity of care.
• Educate patients and families on health management and wellness.
• Maintain accurate, timely, and confidential medical records.
• Respond to medical emergencies with professionalism and urgency.
