About The Position
We are seeking an experienced Licensed Practical Nurse (LPN) for a 13-week contract assignment in a Sub-Acute Rehabilitation or LTACH setting. This role is ideal for LPNs comfortable managing medically complex patients transitioning from hospital to post-acute care. Local and travel LPNs are welcome to apply.
Requirements
- Active LPN license (state or compact as applicable)
- Minimum 1–2 years of recent Sub-Acute Rehab, Skilled Nursing, or LTACH experience required
- Experience caring for medically complex patients required
- Strong clinical assessment and documentation skills
- Ability to function effectively in a fast-paced post-acute environment
Responsibilities
- Provide direct patient care to sub-acute and rehabilitation patients
- Monitor vital signs and patient status, reporting changes promptly
- Administer medications and treatments as ordered
- Assist with wound care, IV therapy (as permitted by scope), and rehabilitation support
- Document patient care accurately and timely
- Collaborate with RNs, physicians, and interdisciplinary care teams
- Maintain compliance with facility and regulatory standards
