Comprehensive orientation: Leadership team is committed to helping staff achieve success and holds a complete and extensive orientation program Located in the heart of Clifton Park, Schuyler Ridge is a 120‑bed skilled nursing facility offering: Sub‑acute rehabilitation Long‑term care Memory care What Sets Us Apart Sub-Acute Rehab Excellence Work on a small Sub-Acute Rehab Unit—a focused care environment that helps prevent burnout and allows you to truly connect with your patients. Long-Term Care (LTC) Designed to Support Nurses Our LTC units are intentionally structured into small, manageable neighborhoods, promoting: Stronger relationships with residents A calmer, more supportive care environment for residents and nursing staff These thoughtfully sized LTC units give RNs and LPNs the opportunity to deliver exceptional continuity of care with the support of a collaborative, engaged team. Dedicated Memory Care Unit Our specialized Memory Care Unit provides: A safe, structured environment tailored to residents living with dementia Smaller unit design to ensure individualized attention and consistent staffing A highly trained team skilled in dementia care best practices This supportive setting enables nurses to provide compassionate, relationship-based care while avoiding the fatigue often associated with large, high-acuity memory care populations.
Requirements
- Current LPN licensure in New York State
- Ability to remain calm, organized, and effective under pressure
- Prior long‑term care experience preferred
- Strong communication and interpersonal skills
- Effective team collaboration abilities
- Solid time‑management and prioritization skills
- A genuine passion for providing respectful, compassionate resident care
Responsibilities
- Provide direct, compassionate nursing care under RN supervision
- Monitor vital signs, administer medications, and assist with treatments
- Support residents with daily living activities and overall well‑being
- Collaborate closely with nursing leadership and interdisciplinary team members
- Communicate effectively with residents, families, and providers
- Document care accurately in electronic medical records
- Maintain safety, cleanliness, and infection control standards
- Serve as a professional, caring representative of the nursing team
