About The Position

"Navitas Healthcare, LLC" is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care during night shifts in a skilled nursing/long-term care environment. In this role, you will monitor resident health, administer medications, and collaborate with the healthcare team to ensure residents receive safe, individualized care.

Requirements

• Active Licensed Practical Nurse (LPN) license in the state of Colorado.
• Previous experience in long-term care or skilled nursing preferred.
• Strong patient assessment, communication, and documentation skills.
• Ability to work night shifts and flexible schedules

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor residents’ conditions and promptly report any changes to the care team.
• Assist residents with activities of daily living while ensuring comfort and safety.
• Maintain accurate and timely patient documentation and medical records.
• Participate in care planning and interdisciplinary team meetings to support resident care plans.
