About The Position

Riverside Lifelong Health & Rehabilitation Mathews is currently hiring for a PRN-C Licensed Practical Nurse (LPN) to join their care team serving the Saluda community. This position will work at least 8 shifts per 4-week schedule, with at least 2 of these shifts being on weekends. This role offers flexibility and pay rates starting at $37.00 per hour! The Licensed Practical Nurse (LPN) works under the supervision of a Registered Nurse, Nurse Manager or Nursing Supervisor, and is ultimately responsible to the Director of Nursing or Assistant Director of Nursing and Director of Quality Care Management. The LPN participates in the assessing, planning, implementation and evaluation of nursing care and provides for the emotional, physical comfort and safety of the residents and their families. The LPN also assists in supervision of auxiliary personnel.

Requirements

• Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire(Required)
• CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)

Nice To Haves

Responsibilities

• Administer medications and treatments accurately and according to provider's orders.
• Allow resident choices within limits of capability considering safety/dignity issues and follow policies and procedures of facility to include resident privacy and dignity.
• Assign duties to non-professional staff such as CNAs according to education, training, policy and resident need in order to assure quality resident care is delivered.
• Engage team members for optimal performance and address performance as needed.
• Develop, implement and evaluate resident care plans.
• Identify changes in condition with proper documentation following the Interact process.
• Demonstrate consistent follow through for positive resident outcomes.
• Utilize good judgment and specialized skills.
