About The Position

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long Term Care (LTC) facility in Rushford, MN. This role focuses on delivering high-quality, patient-centered care while collaborating with residents, families, and the healthcare team to ensure optimal outcomes.

Requirements

• Active LPN license in Minnesota or a compact state
• Minimum of 1 year of experience in Long Term Care or Skilled Nursing Facility (SNF)
• Current BLS certification (American Heart Association preferred)
• Strong clinical judgment, attention to detail, and adaptability to varying schedules

Responsibilities

• Administer medications and treatments as prescribed by physicians
• Monitor residents’ health status and promptly report changes to the RN or physician
• Provide hands-on patient care, including wound care, catheter management, and assistance with activities of daily living (ADLs)
• Accurately document care and patient information in electronic medical records (EMR)
• Collaborate with Certified Nursing Assistants (CNAs) and interdisciplinary team members
• Adhere to infection control standards, safety guidelines, and facility policies
• Offer emotional support and advocate for residents’ well-being
