Department/Unit Summary Transitional Care Management Nurse coordinating care for patients transitioning from inpatient care or facility-based care to the outpatient setting. This role focuses on reducing hospital readmissions ensuring continuity of care and supporting patients during high-risk post-discharge periods in compliance with CMS TCM requirements. Works as an advocate for the physical and emotional well-being of the patient. Requirements

  • Education: Graduate of a Practical Nursing program.
  • Licenses: Current LPN License to practice in the State of Arkansas.
  • Certifications: AHA Basic Life Support (BLS), within 30 days of hire
  • Required Skills: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.
