LPNs working in Dialysis and under the supervision of the RN will be proficient in provides dialytic therapy to patients with acute renal failure or end-stage renal disease (ESRD) and will work closely with the multidisciplinary team to support outpatient hemodialysis on-site. As a Licensed Practical Nurse, you will: Observes safe practices and maintains a safe environment while caring for the patient. Operates the unit communication systems, facilitates unit communication and relays messages appropriately (per HCH standard) including: emergency communications, patient needs, telephone and intercom communications, and other departmental communications. Documents patient care/assessment data in the EMR including but not limited to: vital signs, patient status, treatments, education provided, assessment data, administration and effectiveness of medication, IV therapy.
