A Licensed Practical Nurse, Acute Care Dialysis is responsible for providing direct and indirect patient care to individuals who are receiving dialysis treatment for kidney disease under the supervision of the Dialysis Nurse Supervisor and Charge RN. This includes monitoring vital signs, administering medications, documenting patient information and providing patient education. The LPN must be knowledgeable in the operation of the dialysis equipment and must follow infection control procedures to ensure a safe environment for patients. The LPN must have excellent communication skills. They function as part of the interdisciplinary team and serve as a resource for other teams members.

Requirements

  • Graduate from an accredited Practical Nursing program, required
  • Previous healthcare experience, preferred
  • New Graduates will be considered
  • Current NYS Licensure as a Practical Nurse or valid interim permit, required

Responsibilities

  • Provides direct and indirect nursing care individualized to the patient's needs in the Dialysis setting Collects and records relevant patient health data accurately, with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation and co-worker feedback
  • Identifies and reports patient problems, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation
  • Implements and documents delegated interventions with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation
  • Identifies and reports deviations from expected patient outcomes, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation
  • Provides patients and families with patient education materials, monthly lab information and home medication review per unit guidelines as measured by direct supervisory supervision
  • Performs procedures according to medical orders and nursing standards with appropriate supervision as permitted by law and hospital policy, as measured by direct supervisory observation
  • Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures, 100% of the time, as observed by supervisor and POC staff
  • Maintains and documents testing reagents/supplies and equipment according to established laboratory procedures, 100% of the time, as observed by supervisor and POC staff
  • Consistently documents relevant information in the medical record, including required documentation with RN collaboration
  • Consistently uses aseptic technique when performing Hemodialysis procedures as measured by direct supervisory observation and co-worker feedback
  • Advocates for patients to ensure that care preserves and protects the patient's autonomy, dignity and rights. Communicates effectively with patients, families and co-workers in order to contribute to the effective functioning of the patient care team
  • Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity, 100% of the time, as measured by no significant, substantiated patient or peer complaints received by the manager/supervisor
  • Assures that patients have informed consent signed for indicated procedures, 100% of the time, as measured by random supervisory chart review
  • Demonstrates flexibility and collaboration in solving care problems as measured by direct supervisory observation and co-worker feedback
  • Uses material resources prudently as measured by direct supervisory observation and co-worker feedback
  • Acquires and maintains current knowledge and skills in the safe delivery of dialysis
  • Completes required annual validations according to departmental policy by providing documented evidence of completion to supervisor 100% of the time
  • Participates in 4-8 hours of educational activity related to job function annually and provides documented evidence of attendance or activity to supervisor
  • Practices within the New York State-defined LPN scope of practice 100% of the time, as determined by co-worker feedback and direct supervisory observation
  • LPN provides evidence of current BLS
  • Performs similar or related duties as requested or directed
  • Performs other duties as requested and observed by supervisor or manager.
