Summary Provides a full range of mental health outpatient services including initial assessment, treatment planning, individual, family and group psychotherapy, suicide or other risk assessment, as well as crisis intervention, case management, utilization review, and discharge planning.

Requirements

  • Master’s Degree in Social Work, Counseling, or related mental health field.
  • Demonstrated skill in clinical assessment and therapeutic techniques.
  • Demonstrated knowledge of behavioral health field and resources.
  • LCSW - Licensed Clinical Social Worker - State of Pennsylvania Upon Hire or LPC - Licensed Professional Counselor - State of Pennsylvania Upon Hire
  • Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.

Responsibilities

  • Provides culturally sensitive individual, family, and group psychotherapy in support of the patient's treatment plan monitoring progress toward goals and adjusting treatment approaches, as indicated.
  • Provides psychosocial assessments including clinical impressions and treatment plan, in collaboration with the patient.
  • Monitors for and addresses suicide/high risk as indicated by professional and departmental standards.
  • Provides patient and family education based on treatment needs.
  • Performs case management/utilization review as required by patient need.
  • Provides timely discharge planning with a grasp of LVHN, departmental, and community supports; educates patient/facilitates referrals based on treatment need.
  • Documents services in a timely fashion and in adherence with professional, program, departmental, network, regulatory, and third-party payor standards.
