About The Position

Open Door Health Services is seeking a compassionate and motivated LMHC-A or LSW to join our integrated behavioral health team. Our behavioral health services are embedded within a large primary care practice that has served the community for nearly 30 years. This role is ideal for a clinician who values collaboration, continuity of care, and making a meaningful impact in the lives of patients. We are especially interested in candidates who are looking for a long-term professional home and who are committed to growing their clinical skills while investing in the community we serve.

Requirements

  • Master’s degree in Social Work, Counseling, or a related field required
  • One of the following Indiana licenses required prior to hire: Licensed Mental Health Counselor Associate (LMHC-A) Licensed Social Worker (LSW)
  • Knowledge of clinical documentation and diagnostic standards (ICD)
  • Familiarity with HIPAA regulations
  • Strong interpersonal, communication, and organizational skills
  • Ability to work effectively within a team-based, integrated care setting
  • Commitment to ethical practice, inclusivity, and patient-centered care

Nice To Haves

  • BLS certification preferred (American Heart Association or American Red Cross)

Responsibilities

  • Provide individual and group counseling services to patients with a wide range of behavioral health needs, including anxiety, depression, trauma, and substance use concerns
  • Conduct comprehensive assessments and develop individualized treatment plans based on patient needs and goals
  • Deliver patient-centered, strengths-based therapy in a supportive and non-judgmental environment
  • Maintain timely, accurate, and confidential clinical documentation in compliance with HIPAA and organizational standards
  • Monitor patient progress and adjust treatment plans as clinically appropriate
  • Provide education and referrals to community resources and therapeutic supports
  • Collaborate closely with primary care providers, families, and community partners to ensure coordinated and integrated care
  • Participate in team meetings, case consultations, and supervision
  • Engage in ongoing professional development and training opportunities
