About The Position

"Navitas Healthcare, LLC" is seeking a Licensed Master Social Worker (LMSW) to provide comprehensive social work services within an acute care hospital setting. This role supports patients and families in inpatient medicine, surgery (med/surg), and rehabilitation units by addressing psychosocial needs, discharge planning, and care coordination.

Requirements

• Master’s Degree in Social Work
• Valid Licensed Master Social Worker (LMSW) license issued by NYSED (Primary Source Verification required)
• Minimum of one (1) year of LMSW experience
• Experience in inpatient medicine, surgery (med/surg), or rehabilitation
• Strong communication and interdisciplinary collaboration skills

Nice To Haves

• Hospital experience preferred

Responsibilities

• Conduct psychosocial assessments for hospitalized patients
• Develop and implement individualized care and discharge plans
• Provide crisis intervention and supportive counseling
• Collaborate with interdisciplinary healthcare teams
• Coordinate post-discharge services and community resources
• Maintain timely and accurate documentation
• Participate in multidisciplinary rounds
• Advocate for patients and families
• Ensure compliance with hospital policies and regulatory standards
• Maintain patient confidentiality and ethical standards of practice
