About The Position
"Navitas Healthcare, LLC" is seeking Licensed Master Social Worker (LMSW) for an exciting job in New York, NY.
Requirements
- Master’s Degree in Social Work (MSW)
- Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED) – Primary Source Verification required
- Minimum of one (1) year of experience as an LMSW
- Experience working with adults with mental illness
- Prior hospital experience preferred
Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized treatment plans
- Provide individual and group counseling to adult patients experiencing mental health challenges
- Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care
- Participate in crisis intervention and discharge planning processes
- Deliver case management services and connect patients with appropriate community resources
- Maintain accurate, timely, and compliant clinical documentation
- Educate and support patients and families regarding diagnoses, treatment options, and aftercare planning
- Ensure adherence to hospital policies, state regulations, and professional standards
