This role operates autonomously but requires collaboration with an interdisciplinary team to achieve optimal patient outcomes. Key responsibilities include providing direct clinical care through brief interventions, counseling for primary care patients, warm handoffs, behavioral health consults, individual/couple/family sessions, group visits/support groups, and staff education. The position also involves completing all clinical documentation accurately and timely, providing psychoeducation on mental health and substance abuse disorders, developing and communicating treatment plans to primary care providers, and utilizing data from various sources (situational analysis, functional assessment, clinical review criteria, and EPIC) to identify and prioritize patient supportive care needs. The social worker will also facilitate multi-disciplinary plan of care meetings and initiate referrals to community agencies as needed.
