The Licensed Independent Social Worker utilizes specialized clinical knowledge and advanced clinical skills in psychotherapy, clinical counseling, diagnostic interviews, assessments, and treatment plans for patients ranging in age from adolescent to senior adults. This role operates autonomously but involves collaboration with an interdisciplinary team to achieve optimal patient outcomes. Responsibilities include providing direct clinical care through brief interventions, counseling for primary care patients, warm handoffs, behavioral health consults, individual/couple/family sessions, group visits/support groups, and staff education. The social worker is also responsible for completing all clinical documentation accurately and timely, providing psychoeducation on mental health and substance abuse disorders, developing and communicating treatment plans to primary care providers, utilizing data from various sources (situational analysis, functional assessment, clinical review criteria, and EPIC) to identify and prioritize patient supportive care needs, facilitating multi-disciplinary plan of care meetings, and initiating referrals to community agencies.
