About The Position

The Licensed Independent Social Worker utilizes specialized clinical knowledge and advanced clinical skills in psychotherapy, clinical counseling, diagnostic interviews, assessments, and treatment plans for patients ranging in age from adolescent to senior adults. This role operates autonomously but involves collaboration with an interdisciplinary team to achieve optimal patient outcomes. Responsibilities include providing direct clinical care through brief interventions, counseling for primary care patients, warm handoffs, behavioral health consults, individual/couple/family sessions, group visits/support groups, and staff education. The social worker is also responsible for completing all clinical documentation accurately and timely, providing psychoeducation on mental health and substance abuse disorders, developing and communicating treatment plans to primary care providers, utilizing data from various sources (situational analysis, functional assessment, clinical review criteria, and EPIC) to identify and prioritize patient supportive care needs, facilitating multi-disciplinary plan of care meetings, and initiating referrals to community agencies.

Requirements

Completion of an accredited school of social work with current active registration in the State of Ohio with good standing
Master’s in social work
Licensed as an Independent Social worker in the State of Ohio
Completes 30 contact hours of approved CE during each two year renewal period consistent with requirements of the Ohio Board of Social Work
Maintains continuous certification in American Heart Association’s BLS

Responsibilities

Utilizes specialized clinical knowledge and advanced clinical skills in the areas of psychotherapy, clinical counseling, diagnostic interviews, assessments and treatment plans to patients ranging in age from adolescent to senior adults.
Works autonomously but does collaborate with an entire interdisciplinary team to achieve the best outcomes for the patients.
Performs direct clinical care by providing brief interventions and counseling for primary care patients including warm handoffs, behavioral health consults, individual/couple/family scheduled sessions, group visits/support groups, and education for staff members.
Completes all clinical documentation, i.e. treatment plans, assessments, progress notes, a mandated by regulations and hospital policy for assigned patients accurately and in a timely manner.
Provides psychoeducation regarding a variety of mental health disorders and substance abuse disorders to patients.
Develops a treatment plan for patients and communicates that plan to the patient’s primary care provider.
Utilize data from the situational analysis, functional assessment, clinical review criteria and the electronic medical record (EPIC) to identify and prioritize patient supportive care needs.
Facilitate multi-disciplinary plan of care meetings to maintain rapport and communication with the treatment team to monitor and evaluate the plan of care and patient supportive care plan; identify/prioritize problems and adjust as required.
Demonstrate leadership by initiating referrals to community agencies as needs are assessed (Adult/Child Protective Services, Med Assist, American Cancer Society, DSS etc.).
