About The Position

The C. W. Community Health Center, Inc. has an opening to join our behavioral health department in Charlotte, NC. We are seeking a Licensed Clinical Social Worker (LCSW) to work with our Behavioral Health team as well as with our providers who work in a collaborative environment. The ideal therapist will meet all of the requirements expected of a Licensed Clinical Social Worker (LCSW), including possessing the desire to make an impact on the lives of the patients and families we service.

Requirements

Master's Degree in Social Work
Must possess Licensed Clinical Social Worker (LCSW)
5+ years clinical social work experience
Working knowledge of social theories and practices
Social perceptiveness and empathy
Ability to build and maintain professional helping relationships
Ability to relate and communicate with diverse population and groups
Ability to make psychosocial assessments and develop and implement viable care plans
Excellent listening and communication skills
Experience with electronic health records (EHR)
Ability to provide after-hours service as needed

Nice To Haves

Bilingual preferred
Experience with intimate partner violence counseling highly desired
Athena EHR software experience highly desired

Responsibilities

Consultations about clients
Initial mental health evaluations resulting in realistic treatment plans
Assessing clients using standardized assessment/screening tools, measures, etc.
Provide brief, solution focused therapy
Provide group psychoeducation/treatment related to chronic medical conditions
Responsible for care coordination and referral services
Crisis assessments/triage with appropriate interventions
Provision of other services related to wellness
Documentation of scheduling and services provided in accordance with requirements
