About The Position
Provides clinical case management including ongoing psychosocial assessment and treatment planning for patients within a variety of specialty areas in the hospital. Collaborates with multidisciplinary teams to provide appropriate discharge, ongoing and follow-up plans including the coordination of community and hospital resources and care.
Requirements
- Master's Degree in Social Work
- two years of previous work related experience
- Licensure as a Clinical Social Worker by the State of New Jersey required
