Your Role as a Home Health Licensed Clinical Social Worker

As a Licensed Clinical Social Worker (LCSW), you will provide psychosocial support and behavioral health counseling to patients receiving care in their homes. You will play a vital role in addressing social, emotional, and environmental barriers that impact health outcomes.

Requirements

• Active, unencumbered Florida Licensed Clinical Social Worker (LCSW) license
• Minimum two years of clinical counseling experience
• Valid driver’s license, reliable transportation, and auto insurance

Nice To Haves

• Home health or telehealth experience preferred

Responsibilities

• Provide psychosocial assessments, counseling, and behavioral health support
• Develop and coordinate care plans to address social, emotional, and environmental needs
• Connect patients with community resources and support services
• Assist patients and families in navigating social, financial, and healthcare challenges
• Collaborate with physicians and interdisciplinary care teams to support patient outcomes
• Educate patients and caregivers on coping strategies and available resources
• Maintain accurate and timely documentation in accordance with Medicare guidelines
• Document visits efficiently using AI-assisted EMR tools
