As a Licensed Clinical Social Worker (LCSW), you will provide psychosocial support and behavioral health counseling to patients receiving care in their homes. You will play a vital role in addressing social, emotional, and environmental barriers that impact health outcomes.

Requirements

Active, unencumbered Florida Licensed Clinical Social Worker (LCSW) license
Minimum two years of clinical counseling experience
Valid driver’s license, reliable transportation, and auto insurance

Nice To Haves

Home health or telehealth experience preferred

Responsibilities

Provide psychosocial assessments, counseling, and behavioral health support
Develop and coordinate care plans to address social, emotional, and environmental needs
Connect patients with community resources and support services
Assist patients and families in navigating social, financial, and healthcare challenges
Collaborate with physicians and interdisciplinary care teams to support patient outcomes
Educate patients and caregivers on coping strategies and available resources
Maintain accurate and timely documentation in accordance with Medicare guidelines
Document visits efficiently using AI-assisted EMR tools
