About The Position

We are looking for a Licensed Clinical Social Worker to join a remote care team serving clients through telehealth. This role involves direct therapy, case assessment, treatment planning, and ongoing support for individuals navigating mental health and life stressors. It is a practical, client-facing clinical role with a strong emphasis on consistency, documentation, and sound decision-making.

Requirements

• Master’s degree in Social Work from an accredited program
• Active New Hampshire clinical social work license in good standing
• Ability to independently provide clinical services in a telehealth setting
• ASWB Clinical exam completion is part of New Hampshire’s clinical social worker licensure pathway.

Responsibilities

• Meet with clients virtually for scheduled therapy appointments
• Evaluate emotional, behavioral, and social needs
• Write and maintain individualized treatment plans
• Document session notes and clinical updates in a timely manner
• Support clients with treatment goals and progress tracking
• Identify risk factors and follow proper clinical response procedures
