About The Position

The Level 2 Post Release Services (PRS) Case Manager plays a critical role in providing essential support and services to unaccompanied children and their sponsors. This position requires a high level of technical and professional expertise to provide essential support and advocacy in ensuring the safety and well-being of children. The Level 2 PRS Case Manager primary focus is ensuring the well-being and successful integration of minors into their new environment.

Requirements

  • A bachelor’s degree in social work, behavioral science, or social services with at least two years’ progressive experience in social services, case management, child welfare.
  • Must be skilled at writing professional, thorough assessment reports.
  • Excellent organizational, verbal, written, and interpersonal communication skills, and ability to prioritize duties in a fast-paced environment.
  • Must have a valid driver's license, reliable transportation, insurance, and a clean driving record.
  • The position is remote, and the candidate must have an appropriate workspace to complete documentation, client phone calls, and video conferencing.
  • Proficient in Microsoft Office including using Excel and Teams for maintaining regular contact with supervisor.
  • Ability to type and be computer literate. Comfortable with using a tablet/iPad, or laptop to complete daily assignments is required.
  • Candidate must demonstrate a commitment to comply with mandatory state reporting requirement for child abuse and neglect.
  • Able to work cooperatively with a multi-disciplinary team including project staff, supervisors, remote support staff, community members/organizations and ORR representatives.
  • Able to maintain a cooperative, respectful, and positive disposition always.
  • Possess or be able to obtain CPR and basic first aid certification; and
  • Demonstrated support of Applied intellect’s values by exhibiting respect for others, maintenance of confidential information and an appreciation of a multicultural workplace

Nice To Haves

  • Experience in child welfare, child placements, family preservation and social service provision is desired.
  • Extensive knowledge of immigrant community local resources is desired.
  • Fluency in Spanish is preferred, proficiency in additional languages a plus.

Responsibilities

  • Provide culturally and linguistically appropriate post placement services and comprehensive case management for minors.
  • Carry a case minimum caseload of 15 PRS cases, services are provided virtually and in person every 90 days.
  • Complete individualized needs assessments with minor and sponsor, and develop case plans with particular attention to culture, language, and special circumstances.
  • Provide ongoing case management services to children and families based on their assessed needs.
  • Apply a trauma informed approach to support families in building trust through safety, connection, and self-regulation.
  • Maintain a high standard of record-keeping according to AI and ORR policies and procedures utilizing the UAC Portal and internal software systems.
  • Position requires up to 50 percent long distance travel with limited notice.
  • Provide information, education, referrals, outreach, advocacy, and support to ensure that each child receives the services they require.
  • Assess the safety and appropriateness of the placement using child welfare principles and social work best practices through phone/video calls (using WhatsApp or other video conferencing tool), interviews, and home visits.
  • Maintain confidentiality of health care records and information in keeping with HIPAA and Protected Health Information (PHI) requirements.
  • Must maintain confidentiality in all program areas including information regarding youth.
  • Coordinate and maintain communication with key stakeholders.
  • Participate in regular training and communication sessions utilizing Teams, Zoom or other platforms required for communicating with peers, supervisor, and headquarters staff.
  • Display professional conduct and ensure respectful interactions during any contact with children, coworkers, supervisors, ORR representatives, and other vendors or community members.
  • Adhere to all state and federal laws and regulations.
  • Follow all company policies and procedures.
