Requirements

  • Passion and experience in supporting others ideally in a supported housing setting
  • Level 3 Diploma in Care or equivalent experience (or are willing to work towards it)
  • Experience assessing referrals and coordinating support
  • The ability to work under your own initiative, remain calm under pressure and have a resilient approach
  • You need an Enhanced DBS with barring list (we pay for that)
  • A vehicle insured for business use (and a full UK driving license!), we’ll pay for any business mileage

Responsibilities

  • Lead support planning, risk management and deliver person-centred support to patients who are admitted to Mental Health wards. Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads
  • Ensuring information is logged and recorded accurately on customers support files and systems we use
  • Support Customers with housing, social and financial goals to ensure discharge from hospital is timely.
  • Attend ward meetings relating to customers to ensure communication is efficient.
  • Liaise with TEWV teams and ward staff, and other supporting providers to ensure that support is holistic and appropriate to reduce risk of delayed discharges and readmission.
