About The Position

Provide social services to patients, families and the community as requested or deemed appropriate. Understand and implement social work principles and code of ethics. Conduct discharge planning, crisis counseling, community resource information and education. Assess the patients' services to determine whether they are successful. Work effectively within a multidisciplinary team environment.

Requirements

• Social Worker credentialed from the Texas Department of State Health Services required.
• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree with 1 year of experience OR 5 years of applicable cumulative job specific experience required.

Nice To Haves

• LCSW preferred.
• Experience as a clinician working in a hospital on a interdisciplinary team preferrred.
