About The Position
The Lead Social Worker is responsible for assessing, counseling, intervening, implementing and educating the patient, family/significant others to facilitate movement throughout the continuum of care.
Requirements
- Bachelor’s degree
- 2 –4 years related Experience
- NJ Licensed Social Worker
- Ability to communicate clearly in English, both orally and in writing
- Knowledge of the discharge planning process
- Knowledge of Medicare/Medicaid Regulations; community resources
- Knwolege of the Joint Commission guidelines (continuation of care)
