About The Position

The Lead Specialist,Population Health-Regional serves as additional support for regional providers and CISs to ensure accurate and compliant documentation of patient disease burden in a timely manner. This role will accomplish this in close partnership with regional Population Health Director (PHD), Sr. Clinical Informatics Specialist (Sr. CIS), Practice Managers (PMs), providers (CMDs, SMDs), CISs, and the Central Population Health Team. This role is expected to have deep knowledge of key aspects of Oak Street Health’s clinical documentation processes, including clinical guidelines, clinic workflows, Canopy modules, Daily Huddles, and Clinical Documentation Reviews (CDRs) in order to drive results. They are also expected to understand the impact that diagnosis excellence has on the OSH business model. This role will report to the regional Population Health Director and be responsible for the below:

Requirements

  • 6 month experience working in a healthcare setting
  • Bachelor’s degree required
  • Deep understanding of coding and billing, EMR navigation, Oak Street Health clinical guidelines, morning huddle responsibilities, and CDR responsibilities
  • Eagerness to advance the CIS program's efficiency and effectiveness
  • Strong accountability and discipline against timelines and deliverables
  • US Work Authorization
  • Someone who embodies being Oaky

Nice To Haves

  • Leadership experience and/or natural tendency to lead
  • Comfortable with ambiguity and have a love for problem-solving
  • Basic Excel knowledge and experience

Responsibilities

  • Drive CDR efficiency by reviewing patient charts and identifying relevant medical information in advance of CDR
  • Partner with the Population Health Director to facilitate the execution of backend care report processes to move toward resolution between CDRs, including note amendments and order placement.
  • Lead the Daily Lookback process to review previous documentation, identify actions that need to be taken to align with clinical guidelines, and develop a strategy for completion.
  • Support care teams by providing additional assistance in the review of patient information for new care teams, both new and previous patients under a different provider panel, to identify and bring forward accurate and specific historical information that can help care teams close the knowledge gap.
  • Play a critical role in leveraging data analytics and strategic thinking to identify key clinical documentation insights and drive high-impact clinical documentation initiatives
  • Other duties, as assigned
