About The Position

Homecare Homebase is searching for a Medical Records Services Lead to join our Clinical Operations Services team, helping drive best in class Clinical Operations outsourcing services. This exciting opportunity will include providing outsourced medical records services that drives an ROI for our clients, enhancing the value proposition of the EMR solution, and the Homecare Homebase brand. This will allow the homecare provider to continue to focus on their exceptional patient care services. As Medical Records Services Lead, you will oversee the medical records orders, Face to Face, and supplemental documents processes on the behalf of contracted clients, utilizing HCHB best practice and workflow to ensure that all service level agreements are being met. The Medical Records Services Lead supports this mission by providing careful observation and keen insight into the management, accuracy, and timeliness of all functions of the medical records services division.

Requirements

  • Bachelor's Degree, preferred
  • 3+ years HCHB Home Health medical records processing experience required
  • Management and supervision of medical records and agency office associates
  • Proficient computer skills
  • Knowledgeable in use of Outlook Email, MS Teams, MS Word, MS Excel, MS PowerPoint
  • Knowledgeable in Home Health agency administrative operations and regulatory requirements
  • Experience with HCHB software workflow for medical records and face to face documentation
  • Experience with online e-fax, referral and documentation portals, ie Worldview, Forcura, Wellsky, Careport, Inovolon, Availity
  • Ability to organize and prioritize tasks
  • Ability to communicate professionally, both written and verbal communication
  • Exceptional customer service skills with both internal and external customers
  • Ability to work after hours and weekends, as needed
  • Ability to travel overnight with prior notice, as needed

Responsibilities

  • Administrative oversight of all activities of Medical Records Services department, including participation in team supervision, assessment, onboarding education, and as necessary the performance of all medical records duties as follows:
  • Receive designated orders and documentation from portal, e-fax or agency staff
  • Review and evaluate documentation for appropriateness based on regulatory requirements as well as agency policies and procedures
  • Performance of accurate data entry and document attachment within the patient's medical record
  • Timely completion of all medical records workflow tasks in HCHB, including coordination notes in accordance with agency policies
  • Confirm validity and accuracy of dates, physician signature, and patient specific information
  • Provide timely, updated communications with HCHB leadership, agency leadership, managers and sales as required
  • Initiate communication with physicians to resolve late order retrieval and documentation issues
  • Utilize HCHB analytics and intake dashboard reports as required for development and monitoring KPI
