About The Position

The HeartShare St. Vincent’s Services (HSVS) Nuevo Camino program provides services to 38 unaccompanied minors ages 5–17 in East Brooklyn, offering transitional foster homes, weekday education, and wraparound supports in a trauma-informed, culturally responsive environment aligned with ORR requirements. The Lead Medical Coordinator oversees all health services for children in care, ensuring timely and appropriate medical, dental, mental health, and public health support, while supervising medical coordinators and delivering staff training on pediatric, adolescent, and migrant health topics. This role is responsible for developing and maintaining policies and procedures related to confidential health records, triage and emergency response, environmental health and safety, and post-release care coordination. Additionally, the Lead Medical Coordinator serves as the primary liaison with healthcare providers, insurers, and medical facilities, ensuring full compliance with ORR DHUC policies as well as federal, state, and local regulations including HIPAA and insurance guidelines.

Requirements

  • Bi-Lingual Spanish required
  • Licensed registered Nurse (RN) with a Bachelor’s Or Higher health – related qualification (e.g., Physician Assistant, Nurse Practitioner) required

Responsibilities

  • Serve as a primary point of contact between minors and medical staff and other administrative personnel, including working with case managers to maintain smooth workflow and communication.
  • Ensure the ongoing monitoring of the communication with Point Comfort Underwriters Insurance.
  • Develop, recruit, and maintain positive working relationships between contracted providers to include all health providers and related partners.
  • Oversee and maintain the record-keeping process to ensure there is an effective filing system in which information is forwarded smoothly.
  • Provide ongoing quality assurance practices to monitor and help improve the procedures and ensure high standards of medical care flow and timely distributions to meet mandatory deadlines.
  • Address issues professionally and ensure smooth operation within the medical department.

  • Monitor up-to-date information on transitional youth in care while meeting ORR and Nuevo Camino documentation and reporting requirements.
  • Act as a bridge between healthcare providers and the administrative staff.
  • Periodically travel to health care provider facilities to build positive working relationships and transport/escort clients to medical/mental health appointments.
  • Effectively guide medical staff members and provide support and communicate clearly and frequently, provide supervision and evaluations.
  • Learn Payroll system for approval of timesheet and days off request.
  • Provide ongoing support with continuous feedback and ensure staff understand their duties and demonstrate the ability to communicate the organizational needs to all stakeholders.
  • Participate in required ORR – Nuevo Camino trainings per the federal and state mandates.
  • Oversee, train and mentor support staff; such as medical coordinator and case management staff, ensuring they perform their duties effectively and in compliance with ORR and State policies.
  • Navigate medical systems and provide essential administrative assistance to medical coordinator.
  • Monitor portal entries and medical data collection and entries to portal and medical system NEXGEN.
  • Maintain ongoing monitoring for accurate minors' records to ensure compliance with healthcare regulations and HIPAA standards.
