We partner with Engage Consulting through our rehab provider Concept Rehab; they have been instrumental in improving our quality measures through their expert knowledge. We offer a $10,000 sign on bonus that can be paid quarterly through your first year, or you can opt to be paid $11,500 in one lump sum if you take it at the end of your one year of service. This is a full-time, exempt position with a starting salary of $88,982.40, based on experience.. We are apart of the WI Retirement System (WRS) with matching once vested. We have excellent benefits, a strong reputation in the community, a stellar nursing leadership support team including a day and PM shift supervisor, Rehab Manager, Staff Development Coordinator, Infection Preventionist (Also WCC and completes wound rounds weekly). Our average census runs in the 70’s-80’s and we have 2 full- time MDS staff. The MDS Coordinator is responsible for overseeing the Minimum Data Set (MDS) assessment and care planning process in accordance with state and federal regulations. This role ensures accurate and timely completion of assessments that impact resident care, reimbursement (PDPM), and quality measures. The MDS Coordinator works closely with the interdisciplinary team (IDT) to maintain compliance and promote optimal resident outcomes.

Requirements

  • Valid Registered Nurse (RN) license in the state of employment.
  • Experience in long-term care or skilled nursing setting.
  • Knowledge of MDS 3.0, RAI process, and PDPM.
  • Proficiency in electronic health record (EHR) systems
  • Strong organizational and time management skills.
  • Attention to detail and ability to meet strict deadlines.

Nice To Haves

  • RAC-CT (Resident Assessment Coordinator – Certified) credential through AANAC/AHCA.
  • At least 1 year of MDS coordination experience.
  • Familiarity with quality initiatives such as QAPI and Five-Star Quality Rating

Responsibilities

  • Coordinate and complete timely MDS assessments (initial, quarterly, annual, and significant change).
  • Ensure all MDS data is accurate, complete, and compliant with CMS regulations.
  • Submit MDS assessments through the CMS QIES ASAP system.
  • Collaborate with the Interdisciplinary Team (IDT) to develop and update individualized resident care plans.
  • Lead or participate in resident care plan meetings.
  • Monitor PDPM classifications to ensure accurate reimbursement and documentation.
  • Train and educate nursing and clinical staff on documentation best practices related to MDS and care planning.
  • Conduct audits of clinical documentation to support MDS coding and identify discrepancies.
  • Monitor Quality Measures (QMs) and assist in implementing quality improvement initiatives.
  • Maintain current knowledge of MDS 3.0 guidelines, state and federal regulations, and facility policies.
  • Prepare for and participate in state and federal surveys and respond to deficiencies.
  • Maintain confidentiality of all resident records and information.
  • Maintains current Roster Matrix for the building.
  • Assist the immediate supervisor with various administrative tasks to support department operations as directed: may also be assigned special projects.
  • Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances.
  • May be required to work rotating shifts, holidays, weekends, additional hours, and in different positions as the facility’s needs require.
  • Presents QM data at quarterly QAPI meetings for both long and short stay residents.
  • Completes Facility Characteristic Report.
  • Maintains confidentiality of all data, including resident, employee and operations data.
  • Quality assurance and compliance with all regulatory requirements.
  • Compliance with current law and policy to provide a work environment free from sexual harassment and all illegal and discriminatory behavior.
  • Supports and participates in common teamwork:
  • Participates in paid nursing leadership on call rotation consisting of Fri 5pm-Mon 6am approximately every sixth week. DON takes call Mon-Fri.
  • Cooperates and works together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment.
