Requirements

• Must possess a degree in Social Work
• MSW with 2 years post graduate experience in health care setting
• Minimum of LMSW or LCSW

Nice To Haves

• LCSW-C preferred

Responsibilities

• Providing leadership to the Maternal Child Health (MCH) social work program
• Coordinating the daily huddle and patient assignments
• Coordinating Social Work MCH CEU programs
• Coordinating on-call weekend/holiday MCH social work coverage schedule
• Providing field instruction and clinical supervision
• Providing clinical services including the discharge planning and counseling to MCH patients of all ages
• Developing and implementing an individual plan of care which includes assessment and resource linkage based on needs of patient and family
