About The Position

Under the supervision of the Program Manager, the Lead Coordinator is responsible for supervising the outreach specialist and peer recovery specialist in a “player-coach” model to provide comprehensive support services to individuals experiencing housing transitions with OUD or polysubstance use disorder (including opioids. The Lead Coordinator will support the team to provide screenings, conduct Brief Intervention, provide Referrals to treatment and conduct warm handoffs. The WARM team will provide peer recovery support services using a warm handoff model to individuals with OUD or polysubstance use disorder (including opioids), who are in housing transition.

Requirements

  • Ability to transfer knowledge and skills in areas such as, but not limited to: engagement, developing a therapeutic alliance, harm reduction, motivational interviewing, trauma-informed services, cultural competence, conflict mediation, etc.
  • Flexible to adapt to schedules of work load to the needs and demands of the program
  • Possession of a valid Illinois driver’s license and willingness to use personal vehicle in course of employment, including transporting program participants.
  • Ability to travel to various agency locations throughout the Chicagoland area.
  • H.A.S. requires all staff to work on site.
  • Minimum of a high school diploma or equivalent (GED) is required.
  • Minimum 1 year of management or leadership experience.
  • Minimum 4 years’ experience working in case management and/or outreach setting (personal experience participating in services may substitute for up to 1 year of work experience).

  • Bachelor’s degree, from a regionally accredited educational institution which has U.S. Department of Education approval, is preferred.

Responsibilities

  • Supervise a team of staff, providing guidance on problem solving, crisis management, time management, and client barriers to care.
  • Coach team on harm reduction, motivational interviewing, trauma-informed care, and other evidence-based practices important to street outreach.
  • Train staff on various principles of warm handoff, outreach, HIPAA, and MAR services
  • Oversees and provides an integrated handoff service that include screenings, Brief intervention, referral to treatment, transportation and follow up post discharge for individuals in housing transition.
  • Supports participants as needed with linkages to OP SUD, MAR, and needed community-based care programs
  • Conducts regular visits to identified hospitals and shelters fostering ongoing relationships and developing new linkages as identified in service areas that are needed.
  • Takes the lead in identifying and fostering positive relationships.
  • Establishes weekly staff service schedules at participating sites.
  • Actively participates in relevant networks/coalitions serving participants affected by SUD, SMI, and housing transitions, and represent HAS at community meetings and coalitions.
  • Manages reporting and documentation in CareLogic, and ensures timely staff documentation.
  • Run regular reports to monitor data entry.
  • Monitors grant scopes and collaborates with Program Manager and Development to ensure timely and accurate reporting.
  • Liaises with appropriate community resources on behalf of participants.
  • Performs other duties as requested.
