About The Position

The Lead Community Health Worker (CHW) is a representative member of HCPC’s service area trained to work with clients to provide advocacy, education and direct connections to clinical and social support as well as build community capacity. The Lead CHW should be someone knowledgeable of the community culture and languages spoken, showing a high level of cultural competence, with a goal of providing accurate health and wellness-based information as well as connection to community-based social service organizations in an effort to reduce identified disparities and improve health outcomes. The Lead CHW ensures accurate and updated information is provided to clients and works to directly connect individuals to community services and resources and track outcomes. The Lead CHW works in partnership with the HCPC team to recruit and enroll clients and coordinate care using evidence-based care coordination models and HCPC workflows. Client recruitment may occur through clinical partners, community-based organizations, faith-based organizations, and by participating in public community outreach events. The Lead CHW gathers feedback and recommendations from Senior CHW and CHW regarding advocacy and coordination for clients to clinical and non-clinical service providers, may conduct and/or supervise CHW client home visits and assessments, identifies clients’ needs and priorities, coordinates transportation and access to basic needs, confirms attendance at clinical visits, documents outcomes of clinical and social referrals, and may accompany clients to appointments as appropriate. The client-specific plan for the CHW may be developed in conjunction with the client and the referring partner and is based on evidence-based practices.

Requirements

  • At least 4 years of experience as a Community Health Worker or similar role with demonstrated success and consistency in meeting job expectations.
  • Demonstrated work as a role model and leader as CHW.
  • Work requires at least an associate’s degree with credits in Community Health
  • Knowledge of and a strong desire to help residents of HCPC service area is essential.

Nice To Haves

  • Bachelor’s degree preferred.
  • Bilingual in Spanish or Arabic highly preferred.
