Under the general supervision of the Manager of Pediatric Social Work, the Lead Community Health Worker (CHW) has specialized training and lived experience; operates in a CHW role within an interdisciplinary health care team utilizing an integrated care and treatment model; facilitates effective peer outreach, case management, and navigational services to patients with substance use, mental health and social needs; participates in hiring, training, and supervising CHWs within the specialty clinic they are based; and coordinates collaboration with the medical home-primary care (PCP), community services, case managers, and social workers, to deliver interventions that will address social needs and maximize patient health outcomes.span class=contentcontrolboundarysink Brown University Health employees are expected to successfully role model the organization's values of Compassion, Accountability, Respect, and Excellence as these values guide our everyday actions with patients, customers and one another.
