About The Position

Under the general supervision of the Manager of Pediatric Social Work, the Lead Community Health Worker (CHW) has specialized training and lived experience; operates in a CHW role within an interdisciplinary health care team utilizing an integrated care and treatment model; facilitates effective peer outreach, case management, and navigational services to patients with substance use, mental health and social needs; participates in hiring, training, and supervising CHWs within the specialty clinic they are based; and coordinates collaboration with the medical home-primary care (PCP), community services, case managers, and social workers, to deliver interventions that will address social needs and maximize patient health outcomes.span class=contentcontrolboundarysink Brown University Health employees are expected to successfully role model the organization's values of Compassion, Accountability, Respect, and Excellence as these values guide our everyday actions with patients, customers and one another. Requirements

A bachelor’s degree from an accredited institution is preferred and minimum of 3-5 years relevant professional experience, preferably in a health or substance use treatment setting with an emphasis on populations to be served in the assigned area.
Excellent verbal and written communication skills.
Excellent organizational skills, thorough record-keeping, follow through, and ability to juggle multiple priorities in fast-paced environment with multiple collaborators.
Basic computer skills, including Microsoft Office.
Professional and lived experience in area of specialty preferred.
Prior experience using an electronic health record and producing clinical documentation required.

Responsibilities

Regularly performs all duties of Community Health Worker role.
Responsible for the orientation of new staff including the development of orientation materials and experiences pertaining to the clinic.
Supports staff instruction in the techniques of crises management.
Responsible for designing and implementing in-service programs for new and existing staff.
Participate in ongoing process of consultation with treatment team, family, outside agencies, etc.
Provides regular clinical supervision and work direction to assigned peer staff including Community Health Workers (CHW) and Peer Recovery Specialists within their clinic under the direction of their CHW Supervisor and Manager of Pediatric Social Work.
Is familiar with the workflow of their clinical service site.
Communicates policy and procedures to assigned CHWs and peers within their clinic and participates in the ongoing revision and development of policies and procedures.
In collaboration with Manager of pediatric social work and CHW Supervisors, provides input on hiring, employee work performance and evaluations within their specialty clinic.
May participate in counseling of supervisees around performance issues with support of CHW Supervisor or Manager.
Create a professional environment that supports, stimulates, and challenges each CHW/PRS.
Conduct periodic evaluations of their clinic’s CHW work performance through direct observation, assessments, and reflection and supports development of CHW individualized professional goals.
Serve as liaison between the professional staff and the community; including developing and maintaining relationships with key stakeholders at community organizations.
Maintains records, prepares reports and composes correspondence relative to administrative and/or program management functions at the request of their clinic leadership or the Department Manager.
Provide substance use education and clinical oversight to staff who work directly with patients with co-occurring disorders.
Assisting the multidisciplinary team with brief intervention and substance use interventions
Assessment for substance use treatment and utilization of the ASAM to ensure appropriate treatment is being sought.
Working with a multidisciplinary team inclusive of medical providers, Community Health Workers and Peers, pharmacy, and addiction specialists
From a program development or administrative perspective provides clinical consultation in area of expertise within and outside the hospital.
Participates in and/or supports departmental studies, grants, projects, committees and quality assurance activities.
Maintains and enhances professional expertise through educational activities.
Adheres to Brown University Health, Hospital and departmental policies and procedures to maintain a safe environment of care and be in compliance with all laws, regulations and standards.
