About The Position

The Lead Charge Nurse (LVN/RN) working the 6 PM to 6 AM shift plays a critical role in overseeing and coordinating nursing care during the night hours to ensure patient safety and high-quality clinical outcomes. This position requires leadership in managing nursing staff, facilitating communication between multidisciplinary teams, and responding promptly to patient needs and emergencies. The Lead Charge Nurse is responsible for maintaining compliance with healthcare regulations and hospital policies while fostering a supportive and efficient work environment. This role demands a proactive approach to problem-solving and decision-making to address challenges unique to the overnight shift. Ultimately, the Lead Charge Nurse ensures seamless continuity of care and supports both patients and staff through effective leadership and clinical expertise.

Requirements

  • Current and valid Licensed Vocational Nurse (LVN) or Registered Nurse (RN) license in Texas.
  • Minimum of 3 years of clinical nursing experience, preferably in SNF/LTC.
  • Demonstrated leadership or charge nurse experience during nursing shifts.
  • Basic Life Support (BLS) certification required.
  • Ability to work the 6 PM to 6 AM shift consistently.

  • Experience in a supervisory or management role within a healthcare setting.
  • Familiarity with electronic health record (EHR) systems and clinical documentation software.
  • Strong background in patient safety initiatives and quality improvement programs.
  • Excellent communication and conflict resolution skills.

Responsibilities

  • Supervise and coordinate nursing staff activities during the night shift to ensure efficient patient care delivery.
  • Provide direct patient care as needed, including assessment, medication administration, and monitoring of patient conditions.
  • Serve as a clinical resource and mentor to nursing staff, offering guidance and support in complex patient care situations.
  • Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized patient care plans.
  • Manage staffing assignments, monitor workflow, and address any operational issues to maintain a safe and effective care environment.
  • Ensure compliance with hospital policies, safety protocols, and regulatory requirements during the shift.
  • Respond promptly to patient emergencies and coordinate appropriate interventions.
  • Document nursing activities, patient status, and shift reports accurately and thoroughly.
