Position Summary Copa Health is seeking a Case Manager Lead to participate in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP) and implements services to members in accordance with their plan and provides ongoing care and reviews progress. Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation on members’ progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goal.
