About The Position

The Lead Case Manager provides high-quality, client-centered case management services while offering leadership, guidance, and oversight to other Case Managers. This role ensures service excellence, compliance, and alignment with Hidden Roots’ mission and values. The Lead Case Manager will support individuals and families in accessing resources, navigating systems of care, and achieving improved quality of life outcomes, while also modeling best practices and strengthening team performance.

Requirements

  • Must be open to hybrid setup.
  • Available to work after school hours and weekends as needed
  • Experience in child welfare, childcare, addiction counseling, mental health, or related fields
  • Knowledge of Medicare regulations and services
  • Strong crisis management and intervention skills
  • Demonstrated ability to supervise, mentor, and guide team members
  • Strong organizational and communication skills
  • Reliable transportation, valid driver’s license, and active insurance
  • High school diploma or GED (will be verified)

Nice To Haves

  • Prior leadership or senior case management experience
  • Experience in behavioral health settings

Responsibilities

  • Conduct comprehensive assessments to determine client needs and develop individualized care plans
  • Coordinate, monitor, and evaluate services to ensure client needs are met effectively
  • Provide crisis intervention and support during challenging situations
  • Utilize motivational interviewing techniques to empower clients toward positive change
  • Assist clients with workforce development initiatives and employment readiness
  • Collaborate with community resources and agencies to enhance service delivery
  • Ensure documentation meets agency standards and compliance requirements
  • Provide mentorship, guidance, and support to Case Managers
  • Review case documentation and ensure quality assurance standards
  • Support onboarding and training of new team members
  • Assist leadership with workflow coordination and caseload distribution
  • Promote ethical practice and adherence to agency policies
