Lead Case Manager, Bilingual - Spanish (Enhanced Care Management) This is a full-time, hybrid position that requires a mixture of field work, office work, and time spent working remotely. The primary working location will be the Inglewood and the surrounding area. You have the option to elect to a 9/80 flex work schedule after the first six months of employment. Summary The Lead Case Manager (internally referred to as Lead Care Manager) performs essential functions of care management and care coordination as part of the ECM Program. The Lead Case Manager manages specified cases, coordinates health care benefits, provides education and facilitates member access to care in a timely and cost-effective manner. The Lead Case Manager collaborates and communicates with member, family/support persons, providers, and the ECM team to promote wellness and member empowerment, while ensuring access to appropriate services and maximizing member benefit. The Lead Case Manager serves as a clinical advocate for members, active interdisciplinary team member, liaison with other departments and external health and social service providers.
