About The Position

Peachford Hospital is searching for an experienced Case Manager with leadership experience to work full-time in our Social Services Department as a Lead Case Manager. Social Services Lead Case Managers are empowered to provide quality therapeutic and case management services to our patients using their own personal style, while delivering superior quality care. Requirements

  • A license eligible master's degree in a related clinical area is required.
  • Previous inpatient/outpatient hospital experience is preferred.
  • MSW or other license eligible social services related master's degree.
  • At least 2 years social services/case management experience preferably in a behavioral health facility.

Responsibilities

  • Assist Director of Social Services and Peachford's Management Team in Quality Improvement functions and initiatives.
  • Evaluate need for further training of department staff through audit and quality improvement processes.
  • Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus by interviewing the patient and his/her family members.
  • Provide individual, group and family solutions-focused interventions using appropriate therapeutic techniques and models.
  • Facilitate process-oriented and didactic groups.
  • Document treatment and case management provided to patient in medical record legibly and in a timely manner.
  • Develop and coordinate an individualized discharge plan for the patient by utilizing information obtained from the patient, treatment team and medical record to determine discharge-planning needs.
  • Identify available appropriate family or community resources such as group homes, halfway houses or treatment providers.
