Through its core values of innovation, engagement, and health equity, HCPH strives to bring meaningful solutions to public health issues while keeping Harris County healthy and vibrant Position Overview: The HART Lead Case Manager provides direct support to the case management team. This includes assisting the HART case managers in monitoring, reviewing, and assisting them to implement and execute linkages to services based on the clients' needs. The HART Lead Case Manager will organize, monitor, assist, and ensure that all delivered services by the case management team meet the needs of the clients that are being served by the HART Field Responders. The Lead Case Manager will monitor and provide assistance to case managers and upper management under a combination of administrative/office duties and field work related. The position requires a combination of administrative/office duties, training ability, monitoring audit review readiness, and field/outreach work.

Requirements

  • Bachelor's degree in Social Work, Psychology, Counseling, or a Related Field
  • 5 years of work experience in crisis intervention, case management, or social services, including with vulnerable populations experiencing a variety of challenges including homelessness, mental health, domestic violence, human trafficking, and substance abuse
  • Current Texas Driver's License (upon hire)
  • Knowledge of company protocols, operational procedures, and safety policies.
  • Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook).
  • Excellent verbal and written communication skills.
  • Strong multitasking, organizational, interpersonal, leadership, and problem-solving skills.
  • Demonstrates professionalism and can work well with the public and staff members.
  • Ability to maintain confidentiality when dealing with sensitive information.
  • Ability to de-escalate complex situations by using trauma-informed care and harm reduction practices.
  • Ability to coordinate activities closely with others in stressful situations.
  • Ability to effectively present information in one-on-one and small group settings to other employees and/or managers.

Nice To Haves

  • Master's degree in Social Work, Psychology, Counseling, or a Related Field
  • LMSW (Licensed Master Social Worker), LCSW (Licensed Clinical Social Worker), LCDC (Licensed Chemical Dependency Counselor)
  • Bilingual (English/Spanish) preferred

Responsibilities

  • Monitor and screen daily referrals for eligibility; assign and track case manager caseloads to ensure equitable distribution and timely service delivery. Review and coordinate case management work schedules; evaluate time-off requests with the direct supervisor to maintain adequate coverage and prevent service disruptions, submitting for final approval as required.
  • Review and assist Case Managers in assessing collected data to verify completeness and accuracy of documentation required for program service delivery.
  • Conduct weekly quality assurance reviews of case files to ensure compliance with contract requirements and program standards.
  • Identify areas for improvement and collaborate with the direct supervisor to implement corrective actions, ensuring Case Managers meet established best practices and performance expectations.
  • Support Case Managers in identifying and securing appropriate resources to meet client needs and promote service continuity.
  • Ensure appropriate levels of case management support are provided to help participants achieve solution-focused goals and reduce barriers to long-term stability and self-sufficiency.
  • Build and maintain relationships with key service providers and resource partners to facilitate timely and efficient client linkages.
  • Track key performance indicators (KPIs), analyze client outcomes, and prepare reports to support program evaluation and data-driven decision-making.
  • Stay current on evidence-based practices and emerging trends related to trauma-informed care, domestic violence, human trafficking, and effective resource navigation to enhance program delivery.
