The Lead Case Manager is responsible for providing comprehensive, community-based services to individuals with severe and persistent mental illness (SPMI) and chemical dependency on Guild’s Assertive Community Treatment (ACT) team. In addition to delivering direct client support, managing medications, assisting with benefits, and coordinating care to improve client functioning and quality of life, the Lead Case Manager plays a key leadership role. This includes supporting the Program Manager and Clinical Supervisor with administrative duties such as organizing and managing the team’s calendar, facilitating team meetings, providing guidance to case managers, and assisting with training and development. The Lead Case Manager ensures efficient team operations while maintaining high standards of care and client service.
