About The Position

Are you passionate about making a meaningful impact on community health? We're seeking a dedicated Lead Care Manager to join our dynamic team. As a A CalAIM Justice Initiative Lead Care Manager, you will provide comprehensive care management to re-entry patients with complex needs, effectively coordinating their care across medical, behavioral health, and social services. The Lead Care Manager works closely with the courts, county entities, correctional health, KCS healthcare professionals, community support resources, and the member's family and support network. Requirements

  • Bachelor's degree in social work, public health, healthcare management, or a related field preferred
  • 2+ years of experience providing care management services to individuals with complex needs
  • Excellent interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Proficiency in utilizing motivational interviewing, trauma-informed care, and harm-reduction approaches
  • Competent in Microsoft Office suite (Word, Excel, PowerPoint, or any necessary programs)

Responsibilities

  • Act as the primary point of contact for coordinating comprehensive care across medical, behavioral, and social facets for patients.
  • Conduct initial patient assessments to determine needs and develop personalized care plans.
  • Facilitate communication between patients, healthcare providers, and community resources.
  • Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
  • Develop, execute, and continually refine individualized care plans based on patient needs and preferences.Oversee a caseload of approximately 20-25 patients, or more if deemed appropriate by the CalAIM JI Manager, ensuring all required documentation and intake procedures are completed.
  • Utilize the EHR system and other necessary record-keeping systems for clinical documentation, with a focus on maintaining precision and promptly updating care plan templates and charting.
  • Utilize motivational interviewing, trauma-informed care, and harm-reduction techniques to effectively engage with members and support their well-being.
  • Track and report on patient outcomes and program effectiveness to help improve services.
  • Act as a liaison between members and healthcare providers, community resources, and social services, ensuring their needs are appropriately addressed and advocating on their behalf.
  • CalAIM JI LCMs are scheduled for 8-hour workdays during regular business operations, totaling 40 hours per week. Consistent and punctual attendance is essential to dependable patient care. As part of their salaried roles, LCMs are expected to demonstrate flexibility in their schedules. This includes occasionally working beyond their regular hours to meet patient care demands, respond to emergencies, or attend trainings and conferences. Such flexibility is critical to maintaining high-quality service delivery.
  • Must meet or exceed established health quality metrics, demonstrating consistent improvement in key performance indicators, and may be required to participate in quality improvement initiatives in collaboration with clinical and administrative teams.
  • Must assess and address patient or family questions and concerns, developing a plan to meet their needs.
  • Understands and adheres to regulatory agency standards and organizational policies/procedures, including OSHA, HIPAA, Infection Control, CalOptima, DHCS, and KCS patient care policies/procedures.
  • Remain updated on current best practices in care management to ensure the highest quality of care is being delivered.
  • Perform other duties as assigned or required.
