About The Position
Join Heritage Health Network and make a meaningful impact in your community as a Lead Care Manager supporting members through our Enhanced Care Management (ECM) program. This is a hybrid position (2 days remote, 3 days in the field), giving you the flexibility to work from home while building strong, in -person connections with members across Orange County.
Requirements
- Experience in care or case management (community -based preferred)
- Knowledge of Medi -Cal and ECM strongly preferred
- Strong communication and interpersonal skills
- Ability to work in a team -oriented environment
- Willingness to travel within Orange County
- Bilingual (English/Spanish) preferred
Responsibilities
- Serve as the primary point of contact for members and coordinate their care
- Develop and manage personalized care plans with members and providers
- Collaborate with healthcare teams and community resources
- Support members through education, advocacy, and engagement
- Identify and address gaps in care
- Maintain accurate, timely documentation
