The KC collaborates with the family and other entities to assess and refer them to services such as Family Centered Treatment (FCT), other family preservation models, or kinship care. The KC may carry out aspects of kinship home certification and case management activities for assigned families and youth. The KC always provides individualized, child centered, family focused, culturally competent, strengths based, goal driven and individualized services. The Kinship Coordinator works independently or within a team with minimal supervision and understands community relations as well as Treatment Foster Care (TFC), kinship care and community-based service components.

Requirements

  • Requires a bachelor’s degree in human services (psychology, social work, counseling, criminal justice, sociology, behavioral science, etc.).
  • Requires a valid state driver’s license, safe driving record, and current auto insurance.

Nice To Haves

  • Two (2) years of experience working with at-risk children and families, preferred.

Responsibilities

  • Manages referrals in a timely manner.
  • Completes a comprehensive home and family assessment process while demonstrating cultural sensitivity.
  • Understands core treatment services including FCT, TFC, kinship care, and other in-home and community services in the region thoroughly.
  • Assists families in navigating services which best meet their needs.
  • Involves the family in all aspects of delivery service through consistent communication.
  • Assesses the family’s ability to meet all requirements for providing kinship/foster care within the child serving system.
  • Ensures kinship families moving through the certification process, meet all requirements of state/county regulations, Pressley Ridge service-line requirements and timelines.
  • Provides individualized training and coaching to families, using Pressley Ridge Kinship and Treatment Foster Care trainings and other relevant trainings.
  • Assists with kinship families meeting on-going training and continued certification needs in collaboration with program.
  • Collaborates with identified foster care programs and other service providers.
  • Coordinates care for families with Pressley Ridge programs, other providers and involved system partners.
  • Assists with or complete other projects as requested by members of management.
  • Maintains flexibility and creativity in working a non-traditional schedule to accommodate the needs of children and families.
  • Educates parents and other professionals about the Pressley Ridge values and mission, Kinship Care, and Family Preservation Models.
  • Utilizes data to enhance quality of service delivery.
  • Uses Binti to ensure certification requirements are met, when overseeing kinship home certification.
  • Communicates using professional verbal and written communication.
  • Responds to families as soon as possible and no longer than one day.
  • Receives supervision.
  • Receives supervision and actively communicate with supervisor on an ongoing basis throughout the week by phone, email, written message and when possible, in person regarding program matters.
  • Participate in weekly case review supervision and team meetings as requested.
  • Advocates for and supports kinship families both internally and externally when moving through the certification process and when accessing services.
  • Participates as a member of the program leadership team.
  • Partners with community agencies to improve the referral process and accessibility to services for families.
  • Disseminates agency information and partner with local organizations and individuals who wish to know more about Pressley Ridge services and programs.
  • Represents Pressley Ridge in local, state, and national conferences, seminars, and training by attending or in the role of presenter.
  • Writes clear and descriptive documentation.
  • Completes and maintains thorough documentation of assessment, referrals, case management, and home certification and training activities via direct entry or uploads to appropriate electronic health record.
  • Completes all documentation within required time frames.
  • Assesses risk factors related to the safety and well-being of family members.
  • Reports critical incidents and safety issues to Supervisor.
  • Complies with Pressley Ridge standards, policies, procedures, and state licensing requirements.
  • Completes PR TFC pre-service Training of Trainers training and other assigned training from the Family Centered Treatment Foundation and Pressley Ridge.
  • Attends and participate in all required programs and other professional training.
  • Stays current on changes within Kinship Care.
  • Seeks out resources and learning opportunities to foster ongoing professional growth independently.
  • Attends local, state, or national workshops or conferences periodically.
