About The Position

The Kidney Hero Navigator is responsible for executing the Kidney Navigator Program for patients requiring dialysis during acute hospitalization or at hospital discharge. This role focuses on patient education, referral coordination, patient engagement, and seamless transition to longitudinal kidney care management. The Kidney Hero Navigator enhances patient experience through education, shared decision-making and advocacy, and prepares patients for ongoing management of chronic kidney disease. This position reports to Manager, Kidney Navigation.

Requirements

  • 2+ years combined of related education, experience, or certification
  • Efficient and reliable transportation allowing for the ability to commute to hospitals
  • Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms

Nice To Haves

  • Dialysis Technician certification
  • 1+ years experience in dialysis setting
  • Prior healthcare experience
  • Knowledge of insurance processes and benefits
  • Case management experience

Responsibilities

  • Meets face-to-face with patients requiring outpatient dialysis placement or dialysis education
  • Provides comprehensive education on kidney disease, dialysis treatment modalities, dialysis access options, transplant pathways, and general dietary considerations that align with patient preferences and clinical needs.
  • Collaborates with hospital case managers, social workers, physicians, and interdisciplinary care teams to identify and resolve barriers to timely dialysis placement and prepare patients for dialysis initiation.
  • Acts as a patient advocate by demonstrating empathy, cultural sensitivity, and a strong understanding of individual patient needs while embodying Strive’s core values
  • Provides a thorough handoff to Strive’s Transitions of Care Management (TCM) team by documenting patient-specific needs, preferences, and circumstances within the care management platform (Health Cloud)
  • Ensures a high-quality transition of care by coordinating post-discharge handoff to the patient’s dialysis clinic
  • Serves as a subject matter expert in end-stage kidney disease (ESKD), including dialysis modalities, transplant education, patient engagement strategies, and transitions of care.
  • Promotes patient satisfaction and a positive care experience throughout the referral and placement process
  • Coordinates dialysis therapy placement by referring patients to appropriate local providers based on Nephrologist order/request, geographic proximity, clinical needs, and patient preference
  • Provides HIPAA-compliant renal information and required medical documentation to care teams and dialysis providers while adhering to all policies for secure handling of PHI.
  • Accurately documents referrals and related activities in accordance with program policies and procedures within EPIC
  • Works closely with community kidney care professionals to strengthen existing partnerships and develop new provider relationships
  • Collaborates with care teams to implement best practices for patient transitions across care settings
  • Participates in team meetings with assigned hospital care teams and regional Strive care teams as needed
  • Perform additional duties as required to support program needs
