Requirements

  • Must have a GED or high school diploma.
  • Strong organizational skills with excellent attention to detail.
  • Customer-service–oriented mindset with the ability to communicate professionally and compassionately.
  • Comfortable working in multiple computer systems and digital platforms.
  • Ability to multitask and adapt to changing patient or provider needs.
  • Willingness to travel based on location needs (when applicable).
  • Ability to collaborate effectively with internal teams, physicians, and healthcare partners.
  • Exhibit organizational skills, professional appearance, behavior, and a service attitude toward the community and others.
  • Must possess excellent prioritization and time management skills.

Nice To Haves

  • A bachelor’s degree in a related field is preferred.
  • A nursing background is a plus.

Responsibilities

  • Conduct introductory welcome calls to patients, providing a positive first experience and setting expectations for services.
  • Verify physician signatures on orders and follow up as necessary to ensure timely completion.
  • Maintain ongoing communication with patients regarding their status and care coordination needs.
  • Collaborate with physicians to verify surgical protocols and required documentation prior to patient admission.
  • Provide updates to physicians, providers, hospitals, and other healthcare partners regarding patient status.
  • Fax and email physician orders to obtain necessary signatures.
  • Travel to hospitals, physician offices, or facilities as needed to secure signed orders (location-dependent).
  • In certain cases, travel to meet patients onsite at hospitals or facilities. (location-dependent).
  • Escalate challenges or barriers to the branch location or leadership team in a timely manner.
  • Work collaboratively across departments to ensure smooth patient transitions and resolve issues efficiently.
  • Attend meetings to discuss patient status as well as challenges that need to be addressed.
  • Manage and maintain an up-to-date patient tracking list, including monitoring discharge dates, orders, and location changes.
  • Ensure accuracy and organization of patient records and documentation across multiple computer systems.
  • Support compliance with internal processes and healthcare regulations through accurate data management and communication.
