Key Responsibilities The Platform Coordinator will need to have organizational skills with the ability to effectively prioritize and complete tasks to support Care Coordinators with platform referrals. Coordinates to process patient referrals from Navi, Allscripts, Epic NCH, and Power chart, as well as other referral platforms. Must be detail-oriented, possess the ability to multitask, and be open to cross-functional training in all departments. Displays a high level of patient focus that works directly with Care Coordinators, Case Managers, Physicians/PCP(s), insurance representatives, outside vendors, and other medical personnel. Pulls referrals off platforms to accept/decline, Reviews documents, confirms, and processes the referral through Intake using Pinnacle’s EMR. Responds to Care Coordinator inquiries regarding referral status promptly. Responds to all inquiries with a resolution to concerns and or discrepancies. Coordinates and communicates with the appropriate team regarding any changes or updates to the patient’s payer source on time. Meet productivity goals as assigned by the direct supervisor. Complies with HIPAA regulations on all accounts. All other responsibilities assigned by leadership.
