Assists individuals with learning bus routes to and from work as well as provide support in learning other modes of travel to work. Assesses work strengths and weaknesses and identifies and implements appropriate job accommodations.

Requirements

  • High school diploma.
  • Experience working with individuals with disabilities preferred.
  • We are a drugfree workplace

Responsibilities

  • Coaches individuals served at community job sites by providing orientation and training, assisting with job related issues and demonstrating job functions.
  • Utilize training techniques through learning channels such as verbal prompting, visual prompting, forward chaining, etc. Adapts training techniques based upon participants strengths and needs.
  • Encourages and assists individuals served by teaching job tasks, interpersonal skills, stress management, communication skills, and other job-related activities.
  • Provides support and guidance to individuals completing a community-based work assessment. Identify work skills, strengths, interests, and make recommendations for a realistic vocational direction for the individual served.
  • Identifies and assists employers and individuals served with implementing any needed job modification.
  • Provides problem solving, mediation, and advocacy by teaching individuals served to communicate effectively and appropriately with supervisors and co-workers.
  • Serves as liaison for individuals served with employer, case managers, funding agency, and others by facilitating communication.
  • Provides transportation by driving individuals served to and from work sites for orientation, training, community-based work assessments and other activities as needed.
  • Travel trains individuals served to and from work sites.
  • Keeps accurate records by completing and maintaining required documentation including case notes, monthly summaries and reports, intake forms, activity logs, mileage reports and discharge summaries.
  • Provides coverage at other agency training programs as needed.
  • Maintains knowledge and skills by attending meetings, seminars, trainings, and workshops.
  • Maintains confidentiality of individual case records and other sensitive subject matter.
  • Provides follow along supports to individuals served by performing site visits and contacting employers by phone.
  • Communicates regularly with Easterseals Redwood supervisor and manager regarding progress of individual served.
  • Works assigned schedule and exhibits regular and predictable attendance
  • Works early mornings, evenings, and weekends on occasion to meet workload demands.
