Description Complete all related ISP paperwork, maintain individual's confidential records. Attend ISP's and maintain data systems. Covers classroom and driving as needed.

Requirements

  • Acceptable background check including criminal history background check.
  • Experience and ability to drive 16 passenger size vans.
  • Valid drivers' license, satisfactory driving record, 1 years of driving experience in the United States, and a minimum age of 18.
  • Obtain, and maintain, certification in First Aid, CPR, MAP, NCI, PBS certification and Drivers' Safety and other mandated trainings.
  • Knowledge of DDS and CARF Employment regulations needed.
  • Must be able to work independently while demonstrating effective time management, organizational, and planning skills.
  • Bachelor's degree or equivalence in experience in Human Services.
  • Three years' experience with the DDS ISP system, prior experience with HCSIS preferred.
  • Must have excellent writing skills

Responsibilities

  • Prepare annual update packet for ISP meetings including all required consents,
  • Unique Supports, Search plans and any special protocols.
  • Complete semi-annual progress noted on assigned caseload.
  • Train and supervise staff on ISP goals and data collection.
  • Complete file reviews annually for all Individual Employment Records and request missing information.
  • Transporting individuals to homes, businesses, and agency sites.
  • Immediately reporting unsafe incidents or unsafe working conditions when they occur to your supervisor.
  • Completing assigned work schedules and projects routinely and document completion as necessary.
  • Demonstrating proper attendance.
  • Responding appropriately to emergency situations, following agency policies and procedures.
  • Consistently working in a positive and cooperative manner with fellow staff members.
  • Seeking out opportunities to help rather than waiting to be asked.
  • Considering the impact of your actions on staff members throughout the organization.
  • Recognizing the need for variations in staffing.
  • Demonstrating sound judgment by taking appropriate actions.
  • Taking the initiative to do or redo inadequate or incomplete work, even if its not yours.
  • Ensuring compliance with regulatory standards.
  • Consistently demonstrating the ability to respond to changing situations in a flexible manner in order to meet current needs.
  • Minimizing non-productive time and filling slow periods with activities such as organizing and assisting others.
  • Organizing job functions and work areas to be able to effectively complete assignments within established time frames.
  • Demonstrating knowledge and understanding of all policies and procedures and the ability to reference them from appropriate books and manuals.
  • Complying with infection control policies and procedures in accordance with OSHA's Bloodborne Pathogens standard to ensure a safe working environment for all.
  • Other duties as assigned.
