About The Position

Person in this position will be responsible for providing all intervention services for eligible Veterans. Essential Job Functions include those listed below.

Requirements

  • Must be able to successfully obtain and maintain a criminal background clearance
  • Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required; and
  • CPR Certification, First Aid Certification, and an annual Tuberculosis test.
  • During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
  • Experience consisting of two (2) years of experience in counseling, therapy, prevention services, and/or social work is strongly preferred.
  • Experience working with individuals suffering from mental health and substance abuse disorders is required.
  • GED or High School Diploma is required.

Nice To Haves

  • Military/Veteran preferred.
  • Graduation from an accredited four-year college with a degree in Counseling, Criminal Justice, Social Work, Psychology, or other field related to the position is preferred.

Responsibilities

  • Conduct assessments for all program participants based on their individual strengths, needs, abilities, and preferences (SNAP) on a daily and on-going basis.
  • Conduct a baseline mental health screening for eligible individuals using programs built on proven methods that ensure effective treatment to determine level of suicide risk and validated screening tools that assess behavioral health conditions.
  • Work with local service providers on a day-to-day basis to coordinate services for program participants.
  • Management of the referral process.
  • Provide linkages and a warm hand-off includes providing transportation to local providers, assistance with scheduling appointments, and any other similar activities that may be necessary.
  • Participate in the “train the trainer” training for CALM and provide CALM trainings for community members.
  • Develop an individualized service plan (ISP) and educational program.
  • Provide linkages to the Veteran to the VA or other community-based resources, depending on their preference. Linkage to other supportive services may include primary medical care, testing for HIV and Hepatitis, transportation, employment services, housing assistance, life skills training, parent education, and any other needs the Veteran may have.
  • Establish a regular schedule of contact with each Veteran in their preferred setting (home, local community space, WestCare office, etc.) or via videoconferencing or phone, with additional contact as required or requested by the Veteran. The minimum number of personal interactions with each Veteran will be at least twice per week, possibly more at the beginning of services.
  • Must embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation.
  • Other relevant duties as assigned.
